By providing your insurance information, we can call the company and clarify potential benefits and coverage.
Some plans require pre-approval, others require a Rx (prescription) from a Doctor or Chiropractor. You will be provided with the details specific to your plan.

What Is MEDICAL MASSAGE?
Medical Massage is not necessarily different then any other massage you might receive from me. I will be focusing on the area of concern during this session, but also addressing adjoining areas as we are all connected.
Medical Massage is measured in units of time that are 15 minutes each. I generally can only bill for 4 units (60 minutes) per session. If you desire to have more time during that appointment, you will be responsible for the extra time with payment due at time of service.
Co-pays will be collected at time of service.
Deductibles not yet met will be billed when we receive an Explanation of Benefits (EOB) from your insurance provider. All payments are due at time of receipt. Non payment will result in your dismissal as a client and you will be reported to collections.
Massage Therapist are health care providers, and must comply with HIPAA laws. Your privacy is important to me
* I bill your insurance as a courtesy. No insurance plan guarentees coverare until the claim is paid. You are 100% resonsible any and all services rendered. You will be sent an invoice for any unpaid services.
